I am about to travel to the other end of the earth to bring GPM to the Australasian time zone! My longstanding partnership with Spectrum has lead to this long awaited trip for this project to expand good care for BPD across the lifespan of patients and the complexity of social and clinical variables that need principle driven informed interventions.Good/general psychiatric management (GPM) can fill the gaps of care for those with borderline personality disorder (BPD) as a first step to intervention, alternative to manualized psychotherapies built for only 6-18 months that are either not well suited or effective for the extremes of mild or complex situations, primary care approach, and longer term mental healthcare strategy. People with BPD-- who are by definition in deep pain, struggling to find reasons to stay alive, lacking reasons to feel optimism for themselves or faith in others, and feeling disconnected and alone-- need immediately available treatment practiced in the scope of usual care. Leaving them alone, on long waiting lists, or without options if manualized psychotherapies are a poor fit is iatrogenic and harmful. We cannot rely on the scarcity and short-term, outpatient framework of DBT, MBT or any other psychotherapy alone. If we can fold BPD treatment into the general and good practice of healthcare, a broader proportion of clinicians will feel empowered and equipped to do what they do best in an informed and tailored way. More people will be served and stabilized to start their path of recovery, which usually takes longer than 18 months. For some, help is needed for a long timeframe to build lives that provide purpose, a sense of self, and connection to others; these are the basic solutions to personality dysfunction, loneliness, and poor mental health.
